Register With the State

Once you have been certified by one of our physicians, you will need to apply online for the MMP Program of Illinois. To complete your application you will need your physician’s certification, signed application, photo ID, 2 proofs of residency, along with the state fee. If you are a veteran or enrolled in SSDI or SSI disability programs, the application fee is reduced by half.
